At Eyecare Leaders, we accept FSA accounts and most vision insurance plans. Please call our offices or email us to provide your vision insurance information. Our insurance staff will verify the benefit of your vision coverage with your vision insurance regarding yearly eye examination, contact lenses and glasses.

Policies and Procedures

Our Policy on Confidentiality

We are committed to maintaining the complete confidentiality of our patient’s health care information. As part of our commitment to patient confidentiality:

  • We will not discuss the names of our patients with anyone that is not part of our practice.
  • All information about our patients and their health conditions will be used within our practice in a professional manner.
  • Patient information will never be provided to a third party unless we have the appropriate consent and/or authorization signed by the patient.

Our Procedures

There are many rules regarding the confidentiality of patient information and while our policies and procedures try to anticipate how to comply with these rules, our first and foremost responsibility is to the eye health needs of the patient.

Prior to seeing the doctor on the patient’s initial visit, patients will need to schedule an appointment and complete the patient information form.

When the patient has completed the forms, our staff will review the forms for completeness. We are happy to answer any questions and provide a full and complete explanation of any billing or insurance inquiry.
